Client Forms

Forms can be filled out in person or can be printed and filled out ahead of time.

Due to HIPPA, please do NOT send filled out forms digitally via email/messenger as it is not a privately secure method.

All clients will need to do the INTAKE and AGREEMENT forms.

Other forms are only done as needed.
